Kim, though the combination seems almost infinitely rare, the Colman text does describe a few cases of combined deficiency and also reports experiments linking both PK and MHWK to tissue plasminogen activator (TPA) and urokinase activation subsequent to endothelial cell binding. Thus, though rare, it definitely is a possibility, especially if there is not other explanation. Colman does state the association is experimental and requires more definitive clinical data. Thanks George. This patient actually did have a dRVVT and it was normal. Also, her mixing study showed very strong correction and none of her PTT factors showed any dilutional effect. I did finally find a blurb in a Coleman reference that there may be some association with thromboembolic disease and that both index cases for PK and HMWK dificiency had DVT/PE. Thanks again.
